I keep getting 404 messages when trying to open up documents my applicants have attached.

Also, why are my applicants entering "Shirt Size" when I have hidden this field?

I cloned your form (http://www.jotform.me/form/72335481803455) and noticed that there is a default option selected in the Polo Shirt Size field that's why students were able to bypass the hidden field. To fix this, you need to remove the default option or you may remove the field instead.

Regarding opening the documents, I was not able to replicate the issue. I was able to open the attachments without an issue. Perhaps the issue might be on the form user's end. Could you please ask them to submit the form again and see if you still encounter the issue?
