What would be the reason for an incomplete payment from a form. Obviously the form was complete otherwise I wouldn't see it, so since we are using payola...was it a PayPal issue?
Does the person need to re-submit, or will that produce a double charge?

The reason for an incomplete payment is when the user changes its mind and close the browser before continuing to PayPal login page or if they submitted the form but never paid.
The user needs to re-submit or fill up the form again and complete the payment.
