Why my account is suspended?

    Hello,


    My account is suspended. It is allowing me to change my password but once changed it will not allow me to log in. It says account suspended.


    Please advise.

    Upon review of your account it was found form:

    http://www.jotform.us/form/70993690340158 to be collecting ssn# which is not allowed unless you add a signature widget.


    In conclusion:


    If your purpose is not to collect SSN or driver's license from the general public, but from your own customers; then you need to comply with two things.


     1. Add Terms of Use widget (for your users to read and agree).


    This will surely make your forms not to be considered as guilty of phishing activities


     2. Add a signature widget where your users sign and agree to provide such sensitive information.
