I checked your form and I was able to preview it without having a problem when I click to form URL.
The problem you had experienced could be a temporary issue. Could you please check and let us know if the problem still persists on your side.

Just so you know that this support forum can't accept email attachments. You will have to access this thread and upload it using the image tool. Here's a guide:

As for the problem, did you try using another browser? Can you check with Chrome or Firefox? If you are restricted to using another browser, try this form URL instead;

I would also suggest checking your Firewall and make sure to allow Jotform.
