When I try filling out and submitting the "New Patient Intake" form that I've created, it never goes forward to the thank you page. What Have I done incorrectly?

When I check your form, I see that there are some guilty and phishing fields in your form. I would like to inform you that I have just deleted them immediately. Please note that to collect these kinds of data is strictly prohibited in a plain text.
These fields are against to our Terms of Use. You can only use our payment tools in order to collect payments. If you don't want to collect payments and if you need only payment authorization you can enable the "Authorization Only" feature.

On the other hand, I tried submitting your form and I am unable to reproduce the issue now. Please give it a try and let us know if your problem persists.
