Problems with conditional logic in iframe code

    I am the Research Services Archivist at the University of Kentucky. We are paid subscribers to Jotform and use it for our reference services. Our contact form, which we have used for nearly a year now, relies on several conditional logic statements, which seem to have stopped working as of this morning. Our contact form is located at the URL listed below. This page is our primary point of contact with our patrons so it is urgent that we find out what may have happened to our form. 

    It looks like the problem may be with the iframe code (which we use on our page), and not the embed script. We have to use the iframe for our CMS, though.

    We received an update from our development team and I'm glad to inform you that the issue is already fixed. I have also made some test on my end and I can confirm that it is. However, if you are still experiencing otherwise, please feel free to let us know.

    It looks like whatever was done reset the UniqueID widget, which won't start back at the last used number index

    There is a way to adjust the current index of the Unique ID widget from its properties.

    The index is set at the previous number sequence, but it's ignoring the index. Our Jotform submissions are sent to Asana, where the unique ID becomes part of the task title. As you can see in the comparison below, the index is set correctly but the unique ID is now ignoring the index set in the widget properties:

    I checked your form and I see that the index is changed:

    Also, I checked your last submission and it seems that the index is showing properly:
