I am not able to clone my page for my team member

Hello,

Please assist me.  I have a jotform page. Here is the URL: https://form.jotform.com/72158018296157

This is the page I am attempting to clone for my team mate.  She has joined as a free member.  I have done this for a few other team mates with no problems.

When I clone this page and imput the above url to create this page for her, it does not display the background picture or how this displays in the url.  This is her cloned page after I input the above URL to create the cloned page from a website address:  https://form.jotform.com/73105868677166

I have spent a few hours on it and I am yelling UNCLE!  Please assist me.

What am I doing wrong??

The new user needs to change the form layout from "Single Question in One Page" to "All questions in One Page":
