Hello,
There is an option in the Settings tab of the form called "Form Layout" that allows you to toggle between "All Questions in One Page" and "Single Question in One Page".
I believe selecting "All Questions in One Page" would do in your case.
Please note you need to click "Show More Options" at the Settings page.
There will be a "Layout Change Feedback" dialog once you change your selection. Your feedbacks would also be appreciated.

Thanks for your prompt reply.
However 'Form Layout' options is not available for my form under 'Form Settings'.

Hello again,
We are sorry for this inconvenience.
I re-checked your account and noticed that you are able to create forms in both layouts. That's why you are not able to switch between form layouts. When you want to create form, you will see the following menu:
In your situation, I'm afraid, you need to create form by selecting "All Questions In One Page" option and populate with questions manually.

Hi:
I have encountered the same issue as the person who started this thread ("Form Layout" missing from "Form Settings" )
Even though I have been presented with the option to choose the form layout when I try to create forms, after having selected "All Questions In One Page" the preview and published page still display one question at a time on the page.
Please assist on this issue,
