How to host lightbox form AND script

    I've seen a few threads on actually hosting the javascript for the lightbox feature, but what about the form itself? I already have the form as a php file hosted on my site, but I'm confused as to how to point to that instead of jotform.

    I think your best bet is to use a third-party script to easily display your self-hosted form on a lightbox.

    Try this script:
