    How to host lightbox form AND script

    Asked by CDMjarred on September 12, 2012 at 12:16 PM

    I've seen a few threads on actually hosting the javascript for the lightbox feature, but what about the form itself? I already have the form as a php file hosted on my site, but I'm confused as to how to point to that instead of jotform.

    <script src="/contact_form/js/lightbox.js" type="text/javascript">

    new JotformFeedback({

    formId:'xxxxxxxxxxxxxx',

    base:'http://jotform.us/',

    windowTitle:'Clone of Contact Form',

    background:'#FFA500',

    fontColor:'#FFFFFF',

    type:2,

    height:500,

    width:700

    });

    </script>

    <a class="lightbox- xxxxxxxxxxxxxx" style="cursor:pointer;color:blue;text-decoration:underline;"><img src="../images/icon-free-consultation.jpg" alt="Contact Us" title="Contact Us" border="0"></a>

    Please advise.

    I think your best bet is to use a third-party script to easily display your self-hosted form on a lightbox.

    Try this script:
