I need to implement the old forms

With the current update, the new forms do not like the same to the clients.

I would like to get a template with an old form.

You can copy an old form to your account by cloning it. To clone a form, just follow the instructions provided in this guide: How to Clone an Existing Form from a URL. If you are referring to the old layout, you can change the layout of your form in the Settings > Form Settings > Show more Options > Form Layout and set it to "All Questions in one page".
