We are sorry for the inconvenience you have.
As far as I tested on your end disabling the form named "Calculate Age", I have seen that there is an error with enabling forms back on My Forms page as seen in the following screencast below:
After I have enabled the form back in the form Settings option.
I have seen that there is an error on our side. I have forwarded this issue to our software development team in order to fix it.

Hello Jenny,
Sorry for the inconvenience you've had.
When i checked the situation, i see that "Calculate Age" form belongs to account alexandraparkforms(spollard@alexandrapark.school) which has a Free Plan and already reached the allowed form limit. That's why you, as a subuser, can not enable that form.
You can notify the parent user to upgrade subscription plan or disable a form to make room for any other desired form.
Meanwhile we're working on a error/warning message for the subuser accounts.
