Hi, i have the same problem with my form, i only want file-aceept="pdf,doc,docx)" and it doens aceept files with that extensions, please take a look on that formID=10361047540

I have tested a clone of your orm and it seems to work fine. Please take a look at the thread that you are refering to on your question.
I also noticed that you have a number of other extensions to be accepted on your form. You can edit this as noted on the already metioned thread.
Let us know if you have any other issues!
PS. I noticed that your fomr is hosted on http://yang.jotform.com You may want to log onto that server when you edit your form as we are currently merging to a more powerfull cloud of servers and it has caused some replication delays to some of our users.
