    Hello,

I am the yearbook adviser at Santa Fe High School in Santa Fe, New Mexico. I would like to run a large survey for all students at my school next week. I am willing to purchase a one-month subscription to make this work, but I need to make sure it will do what I want.

First, please see the attached doc with the list of questions. Would this work? Would I get data on all of these questions and all of the answer choices? (I ask this because I have seen Google forms only report the top three choices in a survey with many more choices than three provided, effectively ruining it.)

Second, how is the data presented? Can I import it to Excel? And if I am only paying for a month, do I have access to the data after that month? (It probably won't matter, but just in case I run into a time crunch and don't deal with it right away, I want to make sure the whole thing isn't a bust.)

Third, if I only do one survey, what is the maximum number of responses? Ideally, I would have the capacity of 1,500.

Fourth, I do not want to capture email addresses; it should be anonymous. I assume this is an option?

    Thank you.

Your document didn't come through. Can you upload it to dropbox/google drive and provide the link?

Would I get data on all of these questions and all of the answer choices? (I ask this because I have seen Google forms only report the top three choices in a survey with many more choices than three provided, effectively ruining it.)

Yes all fields will show their values in your submission data. You can even graph this information, say in a pie chart by using a visual report.

Second, how is the data presented? Can I import it to Excel?

Yes you can export your submission data in Excel format.

And if I am only paying for a month, do I have access to the data after that month? (It probably won't matter, but just in case I run into a time crunch and don't deal with it right away, I want to make sure the whole thing isn't a bust.)

Yes you will always have access to your submission data.

Third, if I only do one survey, what is the maximum number of responses? Ideally, I would have the capacity of 1,500.

You would need the Silver plan. It allows up to 10,000 submissions and costs $39 per month.

Fourth, I do not want to capture email addresses; it should be anonymous. I assume this is an option?

Yes you don't have to collect email addresses if you don't want to.

Yes it is possible to answer all of these questions in JotForm.

You can use any of these fields:

    1. Single choice field - to specify only one answer from many

    2. Multiple choice field - to allow the user to check multiple answers

    3. Dropdown field - to allow the user to select one answer from many

OK, I can't afford that. I will get the cheaper version and let it top out at 1,000 responses. Thank you.
