Is it possible to send me the submission as a JPG attachment instead of the PDF?

is it possible to send me the form as a jpg-attachment automatically? At the moment i get the forms as pdf, but it is difficult for to import the form in my dentaloffice software.

I'm sorry, but it's not possible this time. You will have to convert the PDF to JPG manually. There are converters online that you can use. Example, this one:
