My form shows well on desk top but doesn't load on mobile.

I opened the page URL (https://form.myjotform.com/sulaimanxyz/sic-referral-program-partner-signup) you have provided using my mobile device and I was able to replicate the issue. It is showing a blank page.
I will escalate this issue to our developers so they can investigate it. We will contact you again on this thread once we have any updates.
