I can’t open my forms on mobile

hi my form works perfectly in desktop but when it comes to open it in my cell it stacks. I noticed that when I remove the text questions it works. any ideas?

Can you please specify to us what text questions on your form that when removed, the form is working properly on mobile? I checked your form (https://form.jotformeu.com/Ovit/holiday-village-report-archive) on a mobile device and it works fine. I was able to submit the form. Can you please add again the text questions on your form and do get back to us so that we can further investigate the issue?
