Form is gone - Jotform is blank

Hello, 

We recently added our form to Jotform and it was working just fine (11/9/17). However, today (11/10/17), we noticed that our form is no longer present. It is blank - http://prntscr.com/h8tbr6

We have found another instance where this has occurred, however, We have not modified the Jotform code. https://www.jotform.com/answers/1296170-Lightbox-embedded-form-will-not-work

This is currently on the sandbox site as we are working on updating their current site. 

Attached is the error message received.

I can't seem to find any for on the site you shared. It asks for preview code. 

Does your website's has its own lightbox function available so you can try embedding the form inside that lightbox and use iFrame code instead.

The problem should be fixed now but I did not find any lightbox script on the given page (As you may have removed it due to malfunctioning). Can you test it again and get back to us if anything is still wrong?
