Creating a form to accept product trade, swap or sell online

I am looking for a template that would allow clients to input items they have for sale, trade or even give away.  The template would have to include basics like name, contact info, upload picture, and price (if there is one). These would then be uploaded to a Google doc that they could access from a "Swap Meet" link on our website (which I haven't done yet).  Looking at the templates and widgets I don't see anything that would do this.  Ideas?

Thank you for contacting us. Based on the requirements you have I created a sample form where users will fill in their full details and submit the product details including images. i have set the Image upload widget to restrict user to only upload 5 images.
