I was creating a form and got kicked out. It says login failed: autosuspended

    I was creating a form and got kicked out. When I tried to log back in it stated Login failed: AUTOSUSPENDED and I'm using the correct username and password.

    Please advise........

    Your account (bycfinancial) is currently suspended. This form (Credit Sweep Submission Form) triggered the suspension. I checked the form and I can see that there are fields that are asking for Social Security Number and login credentials.

    You are not allowed to collect login credentials here in JotForm. Collecting sensitive information like SSN, Driver's License and Bank information are also not allowed. Please check our terms of use.

    We need to delete the guilty fields first before we can reactivate your account. If that is okay with you, please let us know.

    If you really need to collect this information from the user, then you need to add the following to the form:

    1. An agreement stating the usage of sensitive information. You can use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.

    You can delete the guilty fields that's fine. But the documentation does need to be requested for what there submitting. So with that being said I'll add the terms and conditions widget. But I wasn't finished with the form to do so in the beginning before I was kicked out. So, do I do the widget first before I create the form?
