How can I create a File Upload form that accepts videos and images?

Hi -

I'm considering a form similar to an online incident report or job application template, where I can upload videos, pictures and/or GIFs to different parts of the form. 

I also want to know if you have a feature where a copy of the information inputted can be sent to 2+ email addresses and data collected uploaded to a database.

 Last,

I wanted to add a locator search at the end of the form that searches for the closest hospital, police station or counselor.

If you have the ability to include these features with your forms, what pricing would be best associated with these tools?

I'm considering a form similar to an incident report or job application template, where I can upload videos, pictures and/or GIFs to different parts of the form.

You can add a FILE UPLOAD field to your form that will allow your users to upload any kind of file. You have an option to choose which file types are supported (videos and images) along with the file size limit. The articles I'm linking below will surely help you getting started.
