Do you have an option to add a locator search for hospitals, police stations or counselors

I wanted to add a locator search at the end of the form that searches for the closest hospital, police station or counselor.

I'm afraid we don't have something similar to that. What you're looking for is more of a personalized feature. We have a plethora of widgets that offer location services but these are leaning more towards the location / geolocation of the person viewing the form.
