Why is my account suspended?

I have recently tried logging into my JotForm account and it did not work. When I reset the password and tried to login, it told me the account was suspended. I am trying to set up a registration form for a state parade for the Veterans of Foreign Wars. How can I get this unsuspended?

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Online Foundation Donation Form" has fields asking for customer's credit card number which makes it suspected of being used for phishing.

Please be noted that you should not ask credit card information from your users. If you need to process payments, please use one of our integrated payment tools. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. 

Since you have already deleted your form, I have gone ahead and activated your account. Do try to sign in to your JotForm account and get back to us if you have any trouble. 

Kindly refrain from creating form with email/password, login/password or credit card details.
