the "preferred style of accommodations" box never gets emailed to me. All the other info comes through. I even tried duplicating it or creating a new field and it still won't come through.

I cloned your form and I was able to replicate the issue you are referring. You need to update the email content using the editor.
In the email editor, you can add additional rows for the question/label and the answer. Just click the table and select an option in the toolbar below.
You can use the "Form Fields" drop down to identify the field name. Here's a screen capture:
When you're done, please click the "Save" button.
