I cloned your form and I was able to replicate the issue. I noticed that in the Multiple Choice field there is an extra (") character showing in the inline styling.
I will forward this issue to our developers so they can investigate it further. We will contact you again on this thread once we have an update from our developers.

The problem was related to extra double quotes and it is fixed by now. Your form is rendering as expected. Thank you for reporting the issue to us.
