I was able to replicate the same issue on my end when I tried to clone your form through this URL: https://form.jotform.me/SMFC_AMLA1/SET1

However, when I tried to clone your form through its direct URL where the Form ID is visible: http://www.jotform.me/form/73188457912466

I was not able to replicate it. So, please try to clone your form with this URL. This should work.

On the other hand, I'm escalating this to our development team. Once the problem is fixed, you will be notified via this thread.
