    Hi,

I think I realized why my account has just been suspended. I just created a form to collect international travel information. My company is a start-up firm that is sending touring musicians on different gigs. They've given us permission to gather this information whether it be travel (ie Passport) as well as payment. I'm using it strictly so I can book flights and pay them more efficiently.

Is there anything else I can do? Am I'm going to have to use a different system?

    Thank you,

    Chantel

The reason that your form was disabled and your account automatically suspended was due to the passport number and passport expiration fields on the form. We do not allow for the collection of sensitive personal information such as credit card details, social security numbers (SSN), or passport IDs.

We have an automatic tool that scans for violating forms and suspends the JotForm account that owns them, which is how your account got suspended.

You can collect passenger names, addresses, emails, phone numbers, their citizenship, and other information that is not about their SSN / Credit Cards (without any payment tools) / personal ID.

You have two forms which include guilty fields. To reactivate your account, we need to delete these guilty fields:

Please mention that you agree to our Terms of Use and let us delete these guilty fields.
