i am getting Login failed - AUTOSUSPENDED

Upon review of your account it was found form:

http://www.jotform.us/form/73194230692154 to be collecting credit card information. Which is not allowed under the Terms of Use

You agreed upon acceptance of the Terms of Use that the Company ( meaning JotForm) may terminate your JotForm service immediately if a form's purpose is found to be deceptively obtaining sensitive information, such as and not limited to: credit cards, Social Security Numbers or login credentials, or any other sensitive personal information such as banking sensitive coding.

 Please be aware that this is illegal because it's considered a phishing activity, which violates our terms. Card number, expiration date, security codes and card holder name are considered critical information.
