Autoresponder with the attached file is not sent

I am having problems with the auto respond email with form: https://form.jotform.com/73198601784162

Everything is identical to another form I have that does send the auto response. The only difference with this form is I included a pdf and integrated it with mailchimp. Also when I do the test email, the pdf is not attracted to test, 

Are you referring to the PDF file attached to the autoresponder message? I can see that it has the following PDF file attached to it.

On my first test in your form, I can confirm that the autoresponder email has failed to be sent to my email address.

However, when I cloned the form and make a test submission, I received the autoresponder message with my attached file.

So my suggestion would be to delete the existing autoresponder notification in your form then create a new one and attach the file. See if this makes a difference. If not, let us know so we can check this further.
