If I make any customisation at all to the notification email, images from the Image Upload Preview widget are stripped from the notification email. Display images is turned on.
This is not ideal as I'd like to add some instructions for staff into this email.
Please advise if there is a workaround, or if you can fix this. Thank you.

Just to confirm, are you saying that the image was supposed to be displaying right on the notification you receive in your email? And that stops working as soon as you make some changes to the email template?
I've tested the Image Upload Preview Widget on a cloned copy of your form and on one of my forms but they both end up just showing a link to the image. It doesn't show the actual image itself. Was it working that way before on this form?
Or are you saying the image stops displaying as soon as you edit the submission? I'm asking this too because you encircled it on your screenshot. If there was an instance where the image was displayed on the email template, can you please share a screenshot of it to this thread? Please don't attach it via email.

Thanks for your quick response. Yes the image is supposed to display right on the notification received by email. That stops working as soon as I make any small change to the email template.
Here's a screenshot of the image being displayed successfully when no changes have been made to the email template.

I have tested the issue above and I replicated the same error. I also tested by deleting the notification and adding a new one then submitting form before changing the email template and when I changed the email template the images would not display also.
I have escalated the issue to Level 2 support. They will investigate and post a response here on this thread.

I have checked your form and the emails for replicating the issue. Your problem is related to your Image Upload Preview widget question texts. Your question texts are removed in this widget when unexpected event.
I could see that you have already added a question texts to this widget and your issue was already resolved. We will investigate this issue about removing question text further.

Thank you so much for investigating this. It looks like your theory is correct. Now that there is an actual question for each field, this problem is fixed. Obviously this is a small bug but easy to work around. Thank you. Appreciate your help.
