Format of Form

Hi there,

We have used jot form for about a month. Initially, when we created a form, we were able to have a full form where you could input all information on one page and then click submit.

Now when I create a form, I am only able to have one question, then the user needs to click "next" to go to the next question.

Is there a way that I am able to create a form that is like our original form? Where all information is on one sheet and then they just click submit at the bottom?
