Import Data App: How to import Middle Name, Prefix and Sufix data for Full Name field

    Import data from an excel file to form so it can be edited later using an edit link

    Hello JotForm Support,

    Hope you are well.

    We have a thought of using JotForm to list the data we currently stored for our schools and allow them to make a change if the information we have got is found wrong / out of date. The best solution I have in mind is to fill the form for every school once and send them the edit form path so they can do the change as need. However, with 300+ schools in our system, it doesn't seem efficient compare to our old way using excel. Thus, I wonder if there is any way of loading in a sheet of data (even better if jotform can talk to our database direct) and show the relevant line of data to the preset fields when the representative of the school select their school from the dropdown box (or anything which can identify who they are) so they can change it as needed?

    I understand that it might not be something JotForm exists for / can do but I just want to make sure what I think is not achievable in this awesome tool before moving on to other possible solution.

    What I suggest you do is to import the data from your excel file to JotForm following this guide — Import-App: Easily-import-your-Excel-or-CSV-data-into-JotForm

    You only need to create a form with fields that will match the data in your spreadsheet.

    Once you have successfully imported the data, the data will show in the form's submissions page — How-to-view-Submissions

    Each submission will have a corresponding Submission-ID that you can use to create the edit link which will later be sent to the schools. Edit link format — https://www.jotform.com/edit/SUBMISSION-ID

    Sorry to have taken so long to get back with regards the solution provided. Trialing the csv file I prepared through the instruction, I managed to get what I wanted for most of the fields apart from successfully getting the prefixed "Title" to load. Can you please kindly share the format I should follow to allow the importing Title next to the first and last names?

    It would greatly help if you could share the form you are working on, since import application is suggesting how the names of the fields should look like.

    Hello Nik,

    Thank you for following up. School Directory form stay within my account is the one I am working on and name field is the "Principal Name". I try to follow the format "Title, Last Name, First Name" and "Last Name, First Name, Title" but unfortunately neither is working.

    Unfortunately, this might be not possible to import the Prefix (Title) field using our import app. I have forwarded a ticket to our developers regarding this issue. We will let you know if we have any updates on this.

    It should be possible to import the First Name and Last Name data by following the import app template.

    Example for CSV:

    "20_Principal Name"

    "Doe, John"

    Example for Excel:

    20_Principal Name

    Doe, John
