The following 3 forms are developed using user id: NOELD. Currently the said user id is locked and login is not allowed with reason "Login Failed - AUTOSUSPENDED"
Request your assistance to remove the lock for user id "noeld". The email id is email@example.com
Incase the user id unlocking will take time, please move these forms to my login id.
Kindly treat this as urgent.

The account was suspended because form 73191439029459 "Clone of password reset request" and form 73122685129457 "password reset request" are both collecting usernames.
"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
To resolve this issue you must agree to delete these two forms. Please confirm that you understand and agree. Then we will reactivate your account so that you can log in and delete the forms.
