How do I make a form an app on android phone

I'm afraid we don't have an innate feature/option to achieve that. However, we have an Android Library that you (or your developer) can tinker around with https://developers.jotform.com/libraries#jotform-api-android to check and see if this is something feasible.

On the other hand, the simplest way to achieve this without the need to build your own is to use a 3rd party solution.

If you don't know programming or don't have a developer that works for you, there are plenty of online resources that would allow you to turn a page into an android app. A quick Google search of "turn a webpage into an android app" would yield to some results like:
