I have checked your account and see that there is only one form in the account. You may have created the form by not logging into your account, in which it is created in a guest account. Please provide us with the form ID/URL that is created, so that we'll try to locate the form and assist you further.

That form has been deleted. You have probably accidentally deleted. I cannot find it in your Trash anymore so you have probably deleted it a long time ago.
