How to implement multiple skills question on a job application?

Is there a way to create a skills ability level check box (no experience, beginner, intermediate, advanced) for multiple skills in one question?

I have a dozen or so skills I want to list down and and the applicant can choose the level appropriate to that skill.

Archery               (choose level: NE___   B___ I___  A___)

Canoeing/Kayaking(choose level: NE___   B___ I___  A___)
