Why I'm not able to login AUTOSUSPENDED?

Dear admin,

I'm trying to login with "rameshkumarc08@gmail.com" id for building forms but since few days it's not working properly and showing an error "Login failed - AUTOSUSPENDED".  

Please enable my id so that i can build the forms.

Your account has been auto-suspended by our anti-phishing system because of this form: http://www.jotform.me/form/73167962329466

And the reason is the name of the form you used: "Password Reset". It is highly against our Terms of use to use any kind of password collecting, login forms, dealing with passwords at all. 

So that form's name should be changed or the form removed.

We'll wait for your response.
