Problemi con il modulo

Nell'ultimo passaggio quando clicco su invia mi compare un messaggio di errore " Spiacenti! è permessa solo una voce. (br) Le voci multiple sono disabilitate per questo modulo"

Come posso risolvere il problema?

You are getting this error because you have selected the Check cookies and IP in the Unique Submissions. It checks the cookies and IP so it will not allow if the submission from the same network or the same browsers. 

To solve this, please select No check from the options.
