    It seems to be working fine. I tested this using the form's direct URL and the one you Embedded in your website.

    Sometimes it lag when entering the numbers for birthday, is it because of too many condition logic?

    Yes that might be the possible cause. Not sure what is the use of the other "Secret Code" field .
