    Is it possible to create a inspection type form?

    I would like create a form which has 20 items of text with a box at the side i.e. say the 1st text is (Red) the second is (Blue) etc.  There would be another 18 more all with a box at the side.  If the item was satisfactory then the box would have a tick, if the item was unsatisfactory then a X would be selected.

    If the box had an X a text box would appear and allow comments to be made.  This form data would then need to be collected at a central point and viewed.

     

    Is this possible in jotForm?

    You can create similar form using Matrix field. It is under "Survey Tools" on the left side. However, it would not have the comment box part. You can only have it show checkboxes.
