I save an excel file in web page format: it is stored in my drive: file:///D:/Dropbox/FOS/OPD/lich%20hen/lich%20hen.htm
The purpose is to show that excel file in my form: http://www.jotform.com/?formID=22599262779471
Could you please tell us some way to do that?

You can embed that to your form as iframe using Free Text HTML tool. Example:
<iframe allowtransparency="true" src="http://www.yourwebsite.com/file.htm" frameborder="0" style="width:100%; height:100%; border:none;" scrolling="no"></iframe>
Be sure to replace the bolded URL to your file URL.
