This issue has something to do with how and when Facebook last scraped the URL you're posting and their raw OG tags. To fix the issue, please enter your form link and use the 'Scrap Again' option in the next Facebook tool:
You can also provide us the URL of your form or the Facebook page where you added the form and we will fix the tile for you.
Another way to resolve this issue is to clone the form and use the cloned form.
