How can I make a form with a typed looking report output?

    Hello,

    I am a paramedic on a large department. We recently changed our medical history software. The old software had customizable templates that could output run reports, however the new software does not. I am looking for a solution which can output basically a document (or copyable text) which looks like a hand typed report. I made a quick trial but it seems to just output the form fields in a question/ answer format instead of continuous text such as what I am writing to you right here.

    Do you have a way to make this happen? If your solution works well we could have as many as 200 people we may give access to the software. 

    I attached the template builder we had to give you an idea. everything in [boxes] is a variable drop down which also allows free text. On the right are the variables which are then programmed with options.

    This guide will help you adding the widget to your form: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form 

    The widgets will allow you to have the users to fill the fields and then include the text on your submissions.
