Sorry for the inconvenience that may have caused. Regarding on this issue, is your form already embedded into your webpage? If yes, would you mind if we ask your webpage where your form embedded? However, as I have check your account, I found out that your already exceeded SSL secured submission for free account which may caused account and form disable.
