Our form on our website gives an error message " form disabled " when information is entered into the form. We have also received email from you stating that we are using your free services but we are actully on the $9.95 a month plan.

I noticed that you have used the source code of the form on your website. An error msg such as the one that you have mentioned would come from pointing to a form hosted on our server. You may want to try to embed the form on your site using the iFrame code in order to avoid any conflicts.
As per your account, it is showing as a Premium account on our system.
