Recently I have had 2 people tell me that the acknowledgement boxes have been problematic, causing them to be unable to submit the form.
I did not receive a screenshot but it was the last issue that was described to me was that the text and checkbox for "STATEMENT OF APPLICANT" (at the bottom of the form) was not visible and they could not proceed. Not sure which browser this was in.
Has there been issues with the acknowledgement boxes or signature boxes lately?Page URL:

You must be referring to the Short Scrollable Terms widget on your form.
I have reviewed the widget on the form and test it with my test form as well but so far it was working just fine
Please tell us what specifically is the problem so we can further check. There was no reported issue with the widget as of this checking.
We will wait for your updated response.
