My input table is hidden when no entries are filled in -- how can I get it to show on the PDF attachment even if there is no entry made?

You can go to your submissions page, click the settings button (gear icon on the upper-right part) and uncheck the auto hide empty fields:
This will allow you to show the tables and questions whether or not they are answered.

This still hides entire input tables in the PDF that have no input
The user is forced to enter something in the input table for the table
to show up at all in the PDF

Unfortunately, it seems that empty Input Table fields are not properly included into the PDFs even the Auto Hide Empty Fields option is disabled.
We have forwarded a ticket to our developers regarding this issue. There is no ETA for when it will be addressed, but we will let you know if we have any updates.
In the meantime, it may be possible to manually get the PDFs by printing submissions to the PDF files.

The issue has been fixed now.
Empty Input Table fields included in submission PDFs when the Auto Hide Empty Fields option is disabled on the submissions page. Here is a screenshot of an example PDF:
