The form I'm working on will be used internally. That is, we will fill it out based on data we collect from our customers during an onsite visit. We don't want the form submission to be emailed to the customer, because we want to prepare the data into a report before giving it to the customer. However, we do need to collect the customer's email address in a field on the form.
When I created the form, an autoresponder was automatically set up to email the form submission to the customer's email address. After discovering this, I deleted the autoresponder setting. But twice now, the autoresponder has reappeared on it's own. I haven't even modified the customer email address field.
What should I do about this? I can't check to make sure the autoresponder is still deleted every time I submit the form. How can I be sure it will stay deleted?

The autoresponder notification will be automatically created once you add an email address field in your form. If you have also deleted your email address and added it back, the same thing would happen.
By the way, I couldn't reproduce the problem. After I deleted the autoresponder, it is not added back.
