Form scrolling to bottom when filling the form on iPhones

Hi and thanks for the previous help. My reg form will not work properly on an iPhone. I have done the embed and css for the header (took out the logo) and redid it as per your instructions. 

It still won't work (after clicking on the camp week it takes you to the bottom of the form and if you go back up and put one letter in the name it takes you back to the bottom again etc) it works on android phones, Ipads, and computers. 

I am okay with redoing the whole form if that is what it will take. How can I get it to work on my wix site? You have been very helpful and I appreciate your prompt responses
