We requested a municipal account that was not completed. Can we please make this happen today-

It seems you are applying for a non-profit discount. The JotForm account that was connected to your application was TyngsboroughRec. Please try to re-submit your application with the correct JotForm account (https://www.jotform.com/nonprofit/pricing/). The correct account username is Recreation with email address email@example.com. For the meantime, I have removed the scheduled downgrade on your account so your account will not be disabled.
