    We requested a municipal account that was not completed.  Can we please make this happen today-

    This application is not consider because our sysem do not locate any account TyngsboroughRec neithier account associated with email address apage@tyngsboroughma.gov.

    ACTION REQUIRED:

    1 Please examine the document in the attachment that proves the organization's nonprofit status.
     
    2 If the request is correct, to approve the application please click to "Edit Submission", then change the value on the "Approval" hidden drop-down field from "none" to "Yes".
     
    3 Vice versa, change the related field's value from "none" to "No" to reject the application.
     
    4 If "Approval" field is already "Yes" or "No", this means this application was previously evaluated and user was informed accordingly.
     
    5 Archive this email after you complete the application evaluation.

    Note: You must be logged in to JotForm with your Support account to complete the submission edit operation.

    It seems you are applying for a non-profit discount. The JotForm account that was connected to your application was TyngsboroughRec. Please try to re-submit your application with the correct JotForm account (https://www.jotform.com/nonprofit/pricing/). The correct account username is Recreation with email address apage@tyngsboroughma.gov. For the meantime, I have removed the scheduled downgrade on your account so your account will not be disabled.
