Request to reset my account

    I had to reset my password and now I'm locked out of my account. It is suspended. Please unlock my account.

    We do not allow for the collection of usernames through forms.  Our system also picked up on the Mail Chimp brand name in your form title.  Those two things resulted in our system automatically suspending your account.

    If you would like, we can remove the user ID field from your form and activate your account.

    I updated the form and activated your account.  Your account and forms should be accessible once again.
